We collect your information for the primary purpose of providing a Driving Assessment through one of our Registered Driving Schools. Your basic details will be added to our database and will only be accessed for the purpose for which they were collected and by those involved in providing our service.

You may choose someone to register you on this website on your behalf by providing your contact and other details. The security of this is at your risk.  We will only share your information with health professionals, or relevant parties as part of providing your service.

SENIOR DRIVER ASSESSMENTS (“SDA”, “we”, “us”, “our”) values privacy and is committed to protecting the privacy, confidentiality and security of its clients’ personal information that is under our control. SDA is committed to collecting, using, and disclosing personal information responsibly and only to the extent necessary for the services we provide.

In this Privacy Notice, personally identifiable information about you will be referred to as “Personal Information”. We may collect, use or disclose Personal Information as set out below and as otherwise required or permitted by law. For example, we may collect your name, gender, date of birth, address, email address, phone number, payment card information, physician, diagnosis and other relevant information as required.

Health Care Services: In general, we may collect use and disclose your personal information for the purposes of the driving assessment. We may collect personal information from you directly, or from your medical professional or friends or family who have registered on your behalf on our website. We will only collect the personal information we need to provide you with our services.

In the course of providing our services, your personal information may be shared with the health professional involved in your care (e.g. physician or health care practitioner, an allied professional or member of your clinical care team).

Payment: We may collect, use and disclose your personal information for the purposes of collecting payment for our services.

Legal & Compliance: We may disclose your personal information where required or permitted by law.

We may transfer any information we have about you in connection with a proposed or completed merger or sale involving all or part of SDA or as part of a corporate reorganization or other change in corporate control.
